Very bad cases of hemorrhoids may require a prescription strength medication where the medication is delivered with a suppository or a cream depending on the location of the hemorrhoids. Surgery is the option of last resort and is usually quite rare; surgery is only done if other methods of controlling the hemorrhoids have not worked or if the condition is very painful.
